School Nurse

In order for the School Nurse to communicate with a student's doctor regarding illness, this medical release form must be completed. This form gives authorization for the doctor and School Nurse to disclose protected health, personally identifiable information, and educational records. Medication at School

• Medication must be in the original labeled container as dispensed or the manufacturer's labeled container.
• The medication label must contain the student's name, name of the medication, directions for use, and date .
• This form must be completed each year. In addition, if any changes are made to medications, this form must be completed again.
